'Trick Cyclist' is a term for psychiatrist. He's the one watching closely, playing the music.
The one on the bike pulls his mouth up in a fixed smile, follows the music, AND supports the psychiatrist.
Have you noticed the foot acting as a brake on the wheel? "Keep smiling while I hold you back. You can't see where you're going anyway."
I love this painting. There's a lot of pain behind the circus.
